This policy applies only to information we collect in this App. This policy does not apply to information that:
- We collect offline or on any other Company apps or websites, including websites you may access through this App.
- You provide to, or is collected by, any third party.
Our websites and apps and these third parties may have their own privacy policies, which we encourage you to read before providing information on or through them.

If you do not agree with our policies and practices, do not download, register with, or use this App. By downloading, registering with, or using this App, you agree to this privacy policy. This policy may change from time to time; your continued use of this App after we make changes is deemed acceptance of those changes, so please check the policy periodically for updates.
Children under the Age of 13
The App is not intended for children under 13 years of age, and we do not knowingly collect personal information from children under 13. If we learn we have collected or received personal information from a child under 13 without verification of parental consent, we will delete that information. Children over the Age of 13 and under the Age of 18
This App is only available for individuals aged 13 years or older. If you are 13 or older but under the age of 18, you should review this privacy policy with your parent or guardian to make sure that you and your parent or guardian understand it.
Information We Collect and How We Collect It
We collect information:
- Directly from you when you provide it to us.
- Automatically when you use the App.
Information you provide to us
When you download, register with, or use this App, we may ask you to provide information:
- By which you may be personally identified, such as name, date of birth, postal address, e-mail address, location, profile picture, or telephone number (“personal information”).
- That is about you but does not individually identify you, such as geo-location.
This information includes:
- Information that you provide by filling in forms in the App, including at the time of registering to use the App or reporting a problem with the App.

- Usage Details: traffic data, location data, logs, and other communication data and the resources you access and use on or through the App.
- Device Information: your mobile device and internet connection details, including the device's unique identifier, IP address, operating system, browser type, mobile network information, and telephone number.
- Location Information: real-time information about the location of your device.
- Communications: the App allows for SMS/MMS and secure texting between a patient and provider, and we store any and all communications sent. We are a business associate for the purposes of HIPAA and will enter into a Business Associate Agreement with each covered entity, and with any subcontractors that have access to PHI, including cloud storage service providers.
If you do not want us to collect this information, do not download the App or delete it from your device.
Information collection and tracking technologies
- Cookies (or mobile cookies): a small file placed on your smartphone or computer. You can refuse cookies via your device settings, though this may limit access to certain parts of our App.
- Web Beacons: small electronic files (also known as clear gifs, pixel tags, and single-pixel gifs) that let us count users who have visited certain pages and gather related app statistics.
How We Use Your Information
We use information that we collect about you or that you provide to us, including any personal information, to:
- Provide you with the App and its contents, and any other information, products, or services that you request from us.
- Fulfill any other purpose for which you provide it.
- Give you notices about your account.

- Store information about your preferences, allowing us to customize our App to your interests.
- Speed up communication.
- Locate physicians and availability in your area.
- Recognize you when you use the App.

We may disclose aggregated information about our users, and information that does not identify any individual, without restriction. In addition, we may disclose personal information that we collect or you provide:
- To our subsidiaries and affiliates.

We will not disclose your PHI to any third party unless required by law or to a third-party subcontractor with an executed business associate agreement, and we will not disclose any personal information to any third party for marketing purposes.
Your Choices about Our Collection, Use, and Disclosure of Your Information
- Tracking Technologies: you can set your browser to refuse all or some cookies, or to alert you when cookies are sent. Disabling cookies may make some parts of the App inaccessible or non-functional.
- Location Information: you can choose whether to allow the App to collect real-time location information through your device's privacy settings. Blocking this may make some parts of the App inaccessible or non-functional.
Third-Party Interfacing
We do not sell, market, or otherwise transfer your personal information to third parties. The App does, however, interface with certain third-party technology that allows us to send automatic push notifications alerting you to messages from your care providers, appointments, and similar events. The third-party provider will not have access to your personal information.
Data Security
We have implemented measures designed to secure your personal information from accidental loss and from unauthorized access, use, alteration, and disclosure, and we have assured that our cloud storage provider will provide acceptable levels of security.
The safety and security of your information also depend on you. Where we have given you (or you have chosen) a password for access to certain parts of our App, you are responsible for keeping this password confidential and should not share it with anyone.
As required by various state and federal laws, we may send you notice of security breaches or suspected security breaches that impact your personal information and PHI, to the email address in your account information. Please check your spam filters to confirm emails from us are not blocked.
Changes to Our Privacy Policy
We may update our privacy policy from time to time. If we make material changes to how we treat our users' personal information, we will post the new privacy policy on this page, with the revision date identified at the top. You are responsible for ensuring we have an up-to-date, active, and deliverable e-mail address for you, and for periodically visiting this privacy policy to check for changes.
